Capsular contracture is one of the most common complications associated with breast augmentation surgery. When a breast implant is placed in the body, the immune system naturally responds to the foreign object by forming a protective lining of scar tissue around it. This lining is called the “capsule.”

In most cases, this capsule remains soft and pliable, keeping the implant securely in place while feeling natural to the touch. However, capsular contracture occurs when this scar tissue abnormally thickens and tightens around the implant. 

This compression can lead to breast firmness, changes in the shape of the implant, and in severe cases, physical discomfort or pain. Understanding the causes, recognizing the clinical grades, and knowing the available treatment options is essential for anyone navigating breast implant health.

What Causes Capsular Contracture?

The exact biological mechanism behind capsular contracture is not entirely understood, but it is generally linked to the body’s inflammatory response and fibrotic tissue formation. Medical researchers have identified several factors that can trigger this excessive scar tissue growth:

Biofilm Formation

A biofilm is a microscopic layer of bacteria that can attach to the surface of the implant during surgery. While this bacteria does not cause an active infection with redness or fever, it can trigger a low-grade, chronic inflammatory response that causes the capsule to thicken over time.

Hematoma or Seroma

If blood or fluid collects around the implant post-operatively, the body’s process of absorbing this fluid can stimulate aggressive scar tissue formation.

Implant Rupture

A silent rupture, particularly in older silicone implants, can leak gel into the capsule, causing significant inflammation and capsular tightening.

Surgical Technique

The placement of the implant and the handling of the breast tissue can influence risk. Implants placed under the muscle (submuscular placement) generally have a lower rate of capsular contracture than those placed directly under the glandular tissue (subglandular).

The Baker Grades of Capsular Contracture

To effectively diagnose and treat capsular contracture, plastic surgeons use a standardized clinical classification system known as the Baker Grading System. This system helps medical professionals categorize the severity of the condition.

Grade I: The breast is soft, and the capsule is not palpable. The breast looks and feels completely natural. There is no sign of contracture.

Grade II: The breast is slightly firm to the touch, but there is no visible change in the appearance of the breast. The implant is still in a good position, but the surgeon or patient can feel the tightened capsule.

Grade III: The breast is moderately firm. At this stage, the contracture is not only palpable but also visible. The breast may appear rounded, sit higher on the chest, or look slightly distorted.

Grade IV: The breast is hard, visibly distorted, and the patient experiences physical pain or tenderness when touched or during movement. This is considered severe capsular contracture and typically requires surgical intervention.

How is Capsular Contracture Diagnosed?

If you suspect you are developing capsular contracture, a physical examination by a board-certified plastic surgeon is the first step. The surgeon will assess the softness of the breast, check for visible distortion, and ask about any physical discomfort.

To confirm the diagnosis and rule out other complications like a silent implant rupture, your surgeon may order diagnostic imaging. An ultrasound or an MRI can provide a clear view of the implant shell and the thickness of the surrounding scar tissue. This step is particularly important for patients with silicone implants, as recommended by the FDA, to ensure the structural integrity of the device. Treatment Options for Capsular Contracture

The appropriate treatment for capsular contracture depends on the Baker grade, the patient’s symptoms, and their overall aesthetic goals. While early stages may be monitored, advanced cases usually require surgical correction.

Non-Surgical Management

For early, mild cases (Grade II), surgeons may try conservative measures. Some physicians prescribe off-label medications, such as leukotriene inhibitors (like Singulair), which some studies suggest may help soften scar tissue by modulating the inflammatory response. Additionally, some practitioners use external ultrasound therapy to break up the tightened capsule, though results are highly variable and generally not considered a permanent cure.

Surgical Intervention

For Grade III and IV contracture, or when non-surgical methods fail, surgery is the most effective treatment. The primary surgical treatment is a capsulectomy. During this procedure, the surgeon removes the thickened, hardened scar tissue capsule.

There are two main types of capsulectomy:

  1. Partial Capsulectomy: The surgeon removes only the portions of the capsule that are causing the contracture, leaving the healthy tissue intact.
  2. Total Capsulectomy: The entire capsule is removed. If the implant and the capsule are removed together in one intact piece, it is called an en bloc capsulectomy. This is often recommended if there is a concern about rupture or contamination.

Following the removal of the scar tissue, the surgeon will typically place a new breast augmentation surgery implant. To reduce the risk of recurrence, the surgeon may also change the placement of the implant (for example, moving it from subglandular to submuscular) or use a textured or polyurethane-coated implant, depending on the patient’s specific anatomy and surgical history.

Can Capsular Contracture Be Prevented?

While it is impossible to guarantee a patient will never develop capsular contracture, modern surgical techniques focus heavily on prevention.
